Public Reporting of Concerns or Complaints about Patient Care and Safety

If you, your family, or visitors have concerns or complaints regarding patient safety, we want to hear from you.  We want to take care of any problem as it happens. Our Patient Safety Manager is located on the 2nd floor of the hospital, Room 2201A.  Our Patient Safety Manager is Sheila Anderson, RN.  Phone:  (806) 355-9703, ext. 7553. 

Any individual with a concern about patient/resident care and safety in the hospital should contact the hospital management. If your concern cannot be resolved through the facility, you may contact VA Healthcare - VISN 18 in writing at:
