Telephone Triage and Information Program

The Telephone Triage and Information Program can be reached by calling: (806) 354-7837 or 1-800-OUR-VMED (687-8633)

After placing the call you will receive a menu:

#1 for the RN
#2 for appointment information
#3 for pharmacy information
#4 for Inpatient information

The calls will be answered locally from 8:00am until 4:00pm, Monday through Friday.  After hours EMERGENT medical advice is available with the above phone numbers.  The calls are routed to the Dallas VA.

Canceling or Changing Appointment

If you are calling to cancel or reschedule an appointment at the Thomas E. Creek Department of Veterans Affairs Medical Center or the Lubbock VA Outpatient Clinic, call (806) 355-9703 or 1-800-687-8262.  Choose Option 3 and continue to follow the prompts as instructed.  

If you cannot keep an existing appointment, please contact us as soon as you are aware you cannot make your appointment. This will allow us to reschedule you sooner as well as schedule one of your fellow veterans who may be ill.

Appointment Tips

To make your appointments more productive, we suggest that you arrive 30 minutes before your appointment and bring with you:

• All medications; 
• Home blood pressure records; 
• Diabetes diaries; 
• All outside Medical Records, including those from other VA facilities; 
• Any other information that may apply to your care.
