You have the right to present concerns pertaining to patient care and safety at all Amarillo VA Health Care System locations including Outpatient Clinics. These concerns should be directed to the Manager of the Service or Department that is providing such care. If you are unsure of whom to contact, any employee can direct you to the appropriate supervisor. Should it appear that your concern is irresolvable at this level, you may contact the Patient Advocate located in room 1229 or by calling 806 355-9703, extension 7165.
Any individual with a concern about patient/resident care and safety in the hospital should contact the hospital management. If your concern cannot be resolved through the facility,you may contact VA Healthcare - VISN 18 in writing at:

The calls will be answered locally from 8:00am until 4:00pm, Monday through Friday.  After hours EMERGENT medical advice is available with the above phone numbers.  The calls are routed to the Dallas VA.

If you are calling to cancel or reschedule an appointment at the Thomas E. Creek Department of Veterans Affairs Medical Center or the Lubbock VA Outpatient Clinic, call (806) 355-9703 or 1-800-687-8262.  Choose Option 3 and continue to follow the prompts as instructed.
